# Cigna ABA coverage in Iowa: the intake guide.

_Payer Guide · Cigna · Iowa · Last updated October 2026 · 10 primary sources_

> Evernorth EN0499 criteria; no PA on 97151/97152/0362T; 1–2 hrs supervision per 10 direct; Iowa mandate (no caps from 2026).

Cigna's behavioral health benefits, ABA included, are managed by Evernorth Behavioral Health under coverage policy EN0499 (Intensive Behavioral Interventions, effective May 15, 2026). In Iowa that national policy sits on top of the state autism mandate for large-group and public-employee plans, and plan funding decides whether the mandate applies. Evernorth's administrative guidelines carry an Iowa regulatory addendum, but it covers contract terms (continuity of care, emergency services), not ABA criteria, and it does not apply to self-funded plans.

## Prior authorization and diagnosis at a glance

- **Prior auth for the assessment**: Not required — "Prior authorization is no longer required for assessment ... CPT codes 97151, 97152, or 0362T with a diagnosis of autism," if the provider is independently licensed or a BCBA and the policy covers ABA [2]
- **Prior auth for treatment**: Required — "Prior authorization is typically required for applied behavior analysis"; submit the ABA Prior Authorization Form after the assessment, ideally up to 30 days before or within two weeks after the start date [3][2]
- **Autism diagnosis required?**: Yes — confirmed DSM-5-TR ASD (F84.0–F84.9 except F84.2 Rett syndrome), with the diagnosing clinician's name, credentials and licensure and the date of the most recent diagnosis [1]

## At a glance

- **Covers ABA?:** Yes — for ASD, per Evernorth/Cigna coverage policy EN0499
- **State mandate:** Iowa Code § 514C.31 (large group, more than 50 FTEs, and non-state public employers) and § 514C.28 (State of Iowa employee plan)
- **Mandate age:** No age limit for plans issued or renewed on/after 1/1/2026 (H.F. 330 struck under-19 and under-21 limits)
- **Mandate caps:** None since 1/1/2026: H.F. 330 struck the $36,000/$25,000/$12,500 age-banded ABA maximums; no visit limits; § 514C.22 bars annual/lifetime limits on ASD benefits
- **Exempt from mandate:** Individual and small-group (50 or fewer FTEs) plans are outside § 514C.31; self-funded ERISA plans (preempted)
- **Licensure:** Licensed behavior analyst / assistant behavior analyst under Iowa Code ch. 154D (Board of Behavioral Health Professionals)

## EN0499, applied in Iowa

EN0499 requires a confirmed DSM-5-TR ASD diagnosis "by a healthcare professional who is licensed to practice independently and whose licensure board considers diagnostics to be within their scope of practice," with "the name, credentials, and type of licensure" of the diagnosing clinician and "the date on which the diagnosis was most recently made." The ABA assessment must be done by a BCBA, a Licensed Behavior Analyst or an independently licensed mental health clinician trained in ABA, using a reliable, valid, standardized instrument in its current edition (the policy's example: "must be the Vineland-3 vs. Vineland-II"), "completed within 60 days prior to the start of treatment," with baseline data from the same window. Case supervision must be "consistent with the general accepted standard of care of one to two hours per ten hours of direct treatment," with at least one to two hours a week when direct treatment is ten hours or less. [1][2]

Assessments are easy to start: "Prior authorization is no longer required for assessment ... CPT codes 97151, 97152, or 0362T with a diagnosis of autism," provided the provider is independently licensed or a BCBA and the patient's policy covers ABA. Treatment then needs the ABA Prior Authorization Form; Evernorth encourages requests "up to 30 days in advance of or two weeks after the start date of service." The Autism Care Coordinator team is at 877.279.7603. [1][2]

## The Iowa mandate after H.F. 330

Iowa Code § 514C.31 requires large-group plans (employers with more than 50 full-time-equivalent employees) and non-state public-employee plans to cover ABA for autism under a treatment plan; § 514C.28 does the same for the State of Iowa employee plan. For plans issued or renewed on or after January 1, 2026, H.F. 330 removed the age limits and dollar caps and barred annual or lifetime limits on autism benefits (§ 514C.22). Cost sharing and prior authorization are still allowed; treatment-plan reviews are limited to once every three months in year one and every six months after. Individual, small-group and self-funded plans are outside the ABA mandate. [4][5][11][6]

## Licensure and rates in Iowa

Iowa licenses behavior analysts under Iowa Code chapter 154D on proof of current BACB (or other accredited) certification; technicians are exempt paraprofessionals working under a licensed analyst's direction. Evernorth does not credential technicians: "Evernorth does not credential nonlicensed/noncertified staff. Services for these staff members must be billed under the supervising provider." Commercial rates are negotiated in your Evernorth agreement, which lists the ABA services eligible for reimbursement. [7][2]

## Is Cigna's ABA network open to new providers?

Evernorth Behavioral Health, which runs Cigna's ABA network, says it "is committed to expanding our network of autism providers." It requires providers to be certified by a national governing body or a state licensing board (BCBA, BCBA-D, BCaBA, licensed behavior analyst or other behavioral health licensure). Individual providers complete the Evernorth Behavioral Provider Information Form; autism clinics and large group practices complete the Evernorth Screening Application for Autism Clinics. An application can take up to 90 days, and once a clinic contract is signed each certified or licensed provider must also be credentialed, which "can take an additional 60 to 90 days." Providers must be fully credentialed to render in-network services. "Evernorth does not credential nonlicensed/noncertified staff": technician services are billed under the supervising provider. [2]

## Does Cigna require treatment-plan goals tied to DSM-5 criteria?

Yes — this is the closest any commercial payer in the state comes to a DSM-5 structure. EN0499 (effective 5/15/2026) requires "Clearly defined and measurable goals designed to target specific behaviors and skills across all settings," each naming the target behavior, the measurement method and the mastery standard, and requires that "Treatment goals are directly related to the individual's diagnosis of ASD and the symptoms of ASD defined by the DSM-5-TR (e.g., social communication and social interaction; restricted, repetitive patterns of behavior, interest, or activities)." Goals must come from the full ABA assessment, be developmentally appropriate, and carry dated quantitative baseline data and mastery criteria. The policy ties each goal to the DSM-5-TR symptom domains; it does not prescribe a template that groups them under headings. [1]

## Does Cigna require parent training, and how many hours?

EN0499 requires a "Stakeholder Training" component in the treatment plan but sets no hour minimum or cap. Stakeholder (parent/caregiver, relative, teacher) training "will be conducted by a BCBA®, LBA, or a mental health professional who is licensed to practice independently" and must have "clearly defined, measurable stakeholder goals with mastery criteria," baseline data, a plan to collect progress data, separately documented data for each stakeholder group, and the trainer's name and credentials. Planned group stakeholder training needs its own measurable goals. For continued care, Cigna wants baseline, interim and current data showing stakeholders "continue to actively participate in the treatment" and are progressing toward their goals. The hours approved are set case by case on the authorization. [1]

## Claims: filing deadline and disputes

Cigna’s behavioral benefits, ABA included, are administered by Evernorth Behavioral Health, whose administrative guidelines (revised September 2026) “will only consider claims submitted within 90 days of the date of service or as otherwise defined in your Provider Agreement” — a longer state-law limit overrides it, the clock resets to the date Cigna asks for more information, and Medicaid-secondary claims get three years. Appeals must be started in writing within 180 calendar days of the payment or denial decision. [2][3]

More of Evernorth’s filing rules (administrative guidelines, September 2026): for a secondary claim the 90 days run from "the primary payer’s processing date as stated on an explanation of benefits [EOB] or explanation of payment [EOP]," and the primary EOP goes in with the claim unless the COB data is sent electronically; Medicaid gets three years; a claim returned for more information gets a fresh 90 days from the date of the request. Corrected or replacement claims carry frequency code 7 (replace) or 8 (void) in box 22, not a "Corrected Claim" stamp, and no separate corrected-claim deadline is published. Evernorth publishes no weekly payment cycle or submission cutoff; it offers ERA and EFT "to help reduce your payment cycle." [2][3]

Which codes: Evernorth’s autism resource guide (March 2025) says "All ABA services must be billed with CPT codes 97151–97158, 0362T, and 0373T ONLY," so state HCPCS ABA codes such as H0031, H0032, H2014 or H2019 are not billable to Cigna for ABA. The same guide says "All ABA CPT codes are covered telehealth services." [2][3]

## How fast must Cigna pay a clean claim in Iowa?

Iowa requires insurers and HMOs to “either accept and pay or deny a clean claim” (Iowa Code § 507B.4A), and the Insurance Division’s rule sets the clock: within 30 days after the insurer receives the claim, with any request for information needed to make it clean due within the same 30 days (191 IAC 15.32(2)). A clean claim not paid within 30 days carries 10% annual interest from the thirty-first day. The rule applies to policies issued, delivered or renewed in Iowa. These statutes bind fully insured plans issued in the state; a self-funded employer plan (ERISA) is not subject to them and follows its plan documents and your contract. [9][10]

## What must a Cigna ABA reauthorization request contain, and are graphs required?

Evernorth’s EN0499 (effective May 15, 2026) sets what a continued-treatment request must show: an updated treatment plan covering current deficits and progress in every targeted area; "Quantitative baseline, interim and current data ... with dates reported from when the data were collected" for every behavior and skill, across all settings; current data collected "within no more than 60 days prior to the start date of the continued treatment request" and within 60 days before the request is received; data showing "ongoing and sustained progress"; and a reliable, valid, standardized assessment completed no more than one year before the continued-treatment start date. Where progress is not shown, the request must name the barriers and the protocol changes made. EN0499 asks for dated quantitative data, not for graphs as such; graphs are a common way to present it, but the policy does not require them. [1]

## Intake gates

The questions that decide whether a family can start with Cigna in Iowa, and what they have to bring.

- **Diagnosis recency**: No shelf life on the diagnosis, but its date must be supplied ("The date on which the diagnosis was most recently made"). The clock that matters is the ABA assessment: the standardized instrument must be "completed within 60 days prior to the start of treatment," and "Quantitative baseline data have been collected within 60 days prior to the start of treatment." [1]
- **Who may diagnose**: "A healthcare professional who is licensed to practice independently and whose licensure board considers diagnostics to be within their scope of practice," with "the name, credentials, and type of licensure" provided. [1]
- **Diagnostic tools required**: No named autism diagnostic instrument. The ABA assessment must use "a reliable, valid, and standardized assessment instrument that measures the individual's functioning in the domains included in the diagnostic criteria for ASD," completed in its entirety, by a trained administrator, in its most current version ("must be the Vineland-3 vs. Vineland-II"), with dates, respondents and form type, and standardized scores and tables attached. [1]
- **Telehealth**: "All ABA CPT codes are covered telehealth services" (Evernorth autism resource guide), and EN0499 allows "in-person service delivery, telehealth, or a hybrid," chosen on the individual's characteristics, caregiver participation and environment. Services by telehealth must still meet the direct-treatment definition. That includes the 97151 assessment and 97155 protocol modification. EN0499 describes direct case supervision as occurring "concurrently with the delivery of direct treatment," with the BCBA "face-to-face with the individual and either the Registered Behavior Technician® [RBT®] or the Board Certified Assistant Behavior Analyst® [BCaBA®]," at one to two hours per ten hours of direct treatment; it publishes no separate in-person minimum for supervision delivered by telehealth. [2][1]
- **Other insurance (who pays first)**: Evernorth: "Dependent children of parents who are married and living together follow the 'birthday rule.' The plan of the parent whose birthday falls earlier in the calendar year is primary"; divorced or separated parents follow the "custodial rule" and any court decree. Bill the primary first and submit the COB claim with the primary's payment or denial (paper COB claims need the EOP). Iowa's COB rule (191—38.15) says the same for fully insured plans. With Medicaid, Cigna pays first (42 CFR 433.139); TRICARE pays after ("TRICARE last pay," 32 CFR 199.8); "CHAMPVA is the last payer to OHI" (38 CFR 17.276(d)). [3][13][14][15][16]
- **Age limit** _(plan-dependent)_: EN0499 sets no age limit. The legal layer: plans subject to § 514C.31 or § 514C.28 issued or renewed on or after 1/1/2026 have no mandate age limit; plans renewed earlier keep the old limits until renewal; individual, small-group and self-funded plans follow their plan documents. [4][6]
  - Ask the plan: Evernorth Autism Care Coordinator team 877.279.7603: confirm funding type, group size and renewal date.
- **Referral required?** _(plan-dependent)_: EN0499 states no referral requirement. Where Iowa's mandate applies, the ABA follows a treatment plan developed by a licensed physician or licensed psychologist after a comprehensive evaluation (§ 514C.31). The statute defines "treatment plan" as one "developed by a licensed physician or licensed psychologist after a comprehensive evaluation or reevaluation performed in a manner consistent with the most recent clinical report or recommendations of the American academy of pediatrics." It sets no expiry on the plan, but caps how often the carrier may review it: "not more than once every three months during the first year of the treatment plan and not more than once every six months during every year thereafter," unless the plan and the treating physician or psychologist agree otherwise. [4]
  - Ask the plan: Evernorth Autism Care Coordinator team 877.279.7603 and the member's benefit document.
- **Prior-auth decision time** _(plan-dependent)_: Evernorth asks for ABA requests "up to 30 days in advance of or two weeks after the start date of service"; a later request "may result in a retrospective review and could delay the determination for up to 30 days." The legal decision clock depends on funding: fully insured Iowa plans follow Iowa Code 514F.8 (48 hours urgent, 10 calendar days non-urgent, 15 days complex; approvals valid at least 90 days); self-funded plans follow ERISA (15 days, one 15-day extension; 72 hours urgent). [2][8][12]
  - Ask the plan: Evernorth Autism Care Coordinator team 877.279.7603: ask whether the plan is fully insured or self-funded.

## Delivery and billing rules

Coverage decides whether Cigna in Iowa pays. These decide whether the claim survives: staffing and supervision, concurrent billing, per-day ceilings, who signs the note, where the service is payable, and whose NPI the claim goes out under.

- **Supervision**: Case supervision by a BCBA, LBA or independently licensed ABA-trained clinician: "Direct case supervision (occurs concurrently with the delivery of direct treatment ...) and indirect case supervision is consistent with the general accepted standard of care of one to two hours per ten hours of direct treatment." "When direct treatment is 10 hours per week or less, a minimum of one to two hours per week of direct case supervision is provided." The supervisor's name and credentials must be documented. [1]
- **Concurrent billing (97153 + 97155)**: Allowed for the analyst-plus-technician pair only: "Only one provider can bill for a unit of time, with the exception of CPT codes 97153, 97154, and 97155 (direct supervision when the BCBA/qualified health care provider directs the technician and both are face-to-face with the patient at the same time)." ABA delivered "at the same time as any other treatment modality (e.g., ABA and speech therapy ...)" is not covered. [2][1]
- **Place of service**: Broad. EN0499: "ABA may be provided in any site medically necessary to address individual needs, such as" residential facilities, childcare facilities, homes, schools, transportation, community settings, clinics, vocational or other educational classes, and recreational and social environments. The limit is content, not location: "Services that are considered primarily educational or vocational in nature, or related to academic or work performance are not covered or reimbursable." In academic settings, documentation must show the provider stays in "line of sight, direct engagement" delivering ABA. [1]
- **Bill as provider**: "Evernorth does not credential nonlicensed/noncertified staff. Services for these staff members must be billed under the supervising provider." On paper claims, "List only a BCBA or other licensed provider in box 33." 97151, 97155–97158 are billed by a BCBA-D, BCBA or licensed mental health provider; 97152–97154 may be provided by a BCaBA or technician but are billed by the BCBA-level or licensed provider. [2]
- **Daily limits / MUEs** _(ask the plan)_: Not published as a per-day ceiling. EN0499 sizes intensity to "the severity of the impairments, goals of treatment, and/or response to treatment." Where Iowa's mandate applies (plans issued or renewed from 1/1/2026), visit limits and dollar caps on ABA are barred. [4]
  - Ask the plan: The Evernorth authorization, or the Autism Care Coordinator team at 877.279.7603.
- **Session-note signature** _(ask the plan)_: No ABA-specific session-note signature rule is published in EN0499, the autism resource guide or the administrative guidelines.
  - Ask the plan: Your Evernorth provider agreement and Evernorth Provider Services 800.926.2273.

## Common questions

### Does Cigna require graphs for ABA reassessments?

Not graphs as such. EN0499 requires dated quantitative baseline, interim and current data (current data no more than 60 days old), an updated plan and a standardized assessment no more than a year old; graphs are a common way to show the data, but the policy does not require them.

### Does Cigna accept H0031 or H0032 for ABA?

CPT 97151–97158, 0362T and 0373T only; HCPCS H-codes such as H0031/H0032 are not billable for ABA (Evernorth autism resource guide).

### Does Cigna cover ABA therapy in Iowa?

Yes, for autism under Evernorth/Cigna coverage policy EN0499. Large-group and public-employee plans must cover ABA under Iowa's mandate; individual, small-group and self-funded plans follow their plan documents.

### Does the ABA assessment need prior authorization with Cigna?

No. Evernorth no longer requires PA for 97151, 97152 or 0362T with an autism diagnosis when the provider is independently licensed or a BCBA and the plan covers ABA. Treatment needs the ABA Prior Authorization Form.

### How much supervision does Cigna expect?

One to two hours of case supervision per ten hours of direct treatment, and at least one to two hours a week when direct treatment is ten hours or less.

### Is Cigna accepting new ABA providers?

Evernorth, which runs Cigna's behavioral network, says it is "committed to expanding our network of autism providers." Individual providers submit the Evernorth Behavioral Provider Information Form and clinics the Screening Application for Autism Clinics; allow up to 90 days for the application plus 60 to 90 days of credentialing per provider.

### Does Cigna credential RBTs?

No. "Evernorth does not credential nonlicensed/noncertified staff. Services for these staff members must be billed under the supervising provider." EN0499 expects the direct work from an RBT or BCaBA under BCBA case supervision.

### Does Cigna require ABA goals to follow DSM-5 criteria in Iowa?

EN0499 requires every treatment goal to be "directly related to" the ASD diagnosis and the DSM-5-TR symptoms (social communication/interaction; restricted, repetitive behavior), with measurable targets, baseline data and mastery criteria.

### Does Cigna set required parent training hours in Iowa?

No hour figure is published. EN0499 requires measurable stakeholder (parent/caregiver) training goals with mastery criteria and progress data, delivered by a BCBA or licensed clinician; the hours are set on the authorization.

### What is Cigna’s timely filing limit in Iowa?

Evernorth Behavioral Health, which administers Cigna’s ABA benefit, considers claims submitted within 90 days of the date of service unless your provider agreement or a longer state-law limit says otherwise.

### How fast must Cigna pay a clean ABA claim in Iowa?

Iowa law (Iowa Code § 507B.4A; 191 IAC 15.32) requires a fully insured plan to pay or deny a clean claim within 30 days of receipt, with 10% annual interest after that. Self-funded employer plans are not bound by the statute.

### What documents does Cigna (Evernorth) need for ABA in Iowa?

EN0499 (effective 5/15/2026) requires a confirmed DSM-5-TR ASD diagnosis from an independently licensed clinician whose scope includes diagnosis. The record must give "the name, credentials, and type of licensure" of the diagnosing clinician and "the date on which the diagnosis was most recently made." A provisional or rule-out diagnosis, or an IDEA educational identification, does not count. For treatment, the BCBA's assessment must use a current-edition standardized instrument, completed in full and administered "within 60 days prior to the start of treatment," with score tables. The treatment plan's goals must match the deficits it found.
